Chronic Disease Self Management Program

Coping: A Journey to Living Well - Chronic Disease Self Management Program This free six-week program teaches adults with chronic medical conditions how to manage their health in order to live longer and better lives. Anyone living with a long-term health condition, such as arthritis, diabetes, asthma or high blood pressure is welcome to attend. Family member, friends and caregivers are also welcome. Topics will include: Difference between Acute and Chronic Illness; Making an Action Plan; Using Your Mind to Manage Symptons; Difficult Emotions; Exercise; Nutrition; Making Treatment Decisions: Future Plans for Healthcare and more.
